Yes i agree 2 t bolt clamps i had the regular clamps on myne, had the same problem, went to those no issues as of yet.
Put a few dabs of superglue on the throttle body, then slide the hose over the throttle body. Also upgrade to some good T-bar clamps. Another thing that's also over looked is the bypass big enough?? If the bypass isn't venting enough boost when the TB closes it will shoot off tubes left and right.

usually i weld beads on end of tubing. but if want a really good looking bead can go to hybrid in youngwood and have them bead roll it. i have done that a few times. and def use t-bolt clamps. i usually stock a bunch of them. but think you can get them at hybrid or some napas carry them. also check your local hydrualic store.
You can get really good clamps at Hunter's Truck Sales in fairchance. They are very strong and will not blow off. They have them in a bunch of sizes and are made for big diesel engines.
